After all self-pay services, a superbill will be provided for submission to your insurance company for potential reimbursement. Reimbursement is not guaranteed. HSA/FSA cards are welcome. 

Policies & Payments

Payment: Self-pay payments are due at the time of booking (card, FSA/HSA).
Rescheduling: 48-hour notice required, or a $50 rescheduling/cancellation fee will be invoiced.
